New Client Intake Forms...

Please note that all first time clients must fill out a short intake form prior to any treatment being given.

The brief intake questionnaire is designed to assess any special conditions or concerns of which I should be aware prior to your session. It also inquires as to any special areas of need you would like me to focus on during your massage.

After scheduling an intake form will be emailed to you. Please take the time to fill out the form prior to your session, if you are able to do so. Thank you.
